Binocular Vision Evaluation is an important, but often neglected procedure.
Binocular Vision Test is a very important, but very much neglected assessment of the visual system. Very often especially in children and young adults who have good vision at distance and near have visual symptoms of headaches, eye strain and watery eye, which go undetected unless tedious Binocular Vision tests are performed.
This assessment is to simply evaluate how the two eyes team together in terms of accommodation and convergence, and the ability of the ocular muscles to fuse objects with ease and to rule out any stress on the system. If found to be somewhat defective, eye exercises in most cases or lenses will offer relief.

Ortho-K (Orthokeratology) or Eye Braces is a non-surgical procedure that gently shapes the curvature of the cornea to improve vision with the help of a customized contact lens
Simply speaking, Orthokeratology (Ortho-K) is a contact lens that you wear so you do not have to wear contact lenses — During your waking hours, that is. Confused, well let me be more clear. This is a night time lens, worn overnight, it gently molds the shape of your cornea, causing it to flatten, thus changing the refractive state of your eyes. This flattening is similar to the flattening done with a laser in a procedure called Lasik.

The Topograph is today an indispensable tool in any Hi Tech Eye Clinic, dealing with state-of-the-art customised contact lens design.
It provides a useful data of the shape and form of the corneal surface, including the presence of optical distortions. These lenses are fitted and dispensed for diseased corneas such as Kerataconus.
Customised RGP Multifocal contact lenses and the prescribing of lenses for Orthokeratology which requires the information this machine provides, thus ensuring a successful outcome of these lenses.

The term refraction means to bend light. Your optometrist will use several methods to bend or change the direction of light coming from a distant object, to focus accurately on the retina by introducing different lenses before your eyes. When this is achieved, vision will be clearest. This results in the prescription for your spectacles or contact lenses. Refraction can only be accurate if the letter chart is at least 6 meters or 20 feet away from the patient. Any shorter, and the accuracy drops. To save space, most practitioners will use a mirror to double the distance.

Glaucoma presents in many forms. Most common are open and closed angle Glaucoma. Intraocular Pressure (IOP) in excess of 21mmhg, is a sign that requires close monitoring. Observation of the optic nerves is a must during any eye examination as low IOP can also result in Glaucoma.
Closed Angle Glaucoma is the narrowing of the drainage channel which obstructs the release of Aqueous (fluid in the eye) resulting in an acute rise of eye pressure, which can quickly damage the optic nerve and vision. It is easily detected on eye examination by an Optometrist or Ophthalmologist and very easily treated, provided it is detected in time. If left untreated it will result in irreversible vision loss.
Secondary open angle Glaucoma is a result of other systemic medical conditions such as Diabetes.

Uncontrolled Diabetes is a medical condition which can cause loss off vision through different stages of the disease. Evaluation off the Retina through Fundus imaging is an important tool in the screening process for the condition.
Diabetic retinopathy is the leading cause of blindness in the world. If you are suffering from diabetes, it is important that your blood sugar level is controlled. You should also have a dilated eye exam by an ophthalmologist at least once a year.
Diabetes affects the blood vessels in the eye, these damaged vessels will leak some blood fluids onto the retina. when the disease becomes more severe, new blood vessels will begin to grow. These new vessels are fragile and bleed easily causing damage to the underlying retina. In some cases scaring can cause the retina to detach.

Visual Field testing is an important diagnostic tool in the assessment of the eye’s visual fields. A normal visual field as measured from the central fixation point is 90 degrees temporally, 50 degrees nasally, 50 degrees superiorly and 60 degrees inferiorly. Many eye conditions can cause a narrowing of the field of vision, examples are Retinitis Pigmentosa, Glaucoma, etc. Other conditions for example Macular degeneration will only affect central vision. Even space occupying lesions, for example tumors in the brain may cause abnormal patterns in the visual fields.
When looking straight ahead, you should be aware of what objects are to the right and left of your fixation point. If you are approached from the side by a friend, you should be aware of his approach. This awareness is your field of vision.
Most patients are unaware of reducing field of vision as this happens so gradually. Another reason is what one eye missed, the other eye sees.

Optical Coherence Tomography Analysis is much like Ultra-Sound, except that it uses light instead of sound This instrument is able to make 3-dimensional images of the Retina. The layers that make up the retina are easily visualized, the location of any abnormalities within the retina can then be identified. Eye conditions, for example Central Serous Retinopathy, Glaucoma may be diagnosed and monitored.
Picture on the right shows the layers of the retina, The dip in the center is the Fovea, the most sensitive area of the retina. this is your central fixation spot. images projected here is the sharpest. Look towards the left of picture A. notice that a separation of the layers has occurred. this is Central Serious Retinopathy which may be difficult to pick up from a picture of the retina.

The Optos Daytona was designed to assist eye care professionals to diagnose and manage diseases of the retina. It produces a 200 degree image of the retina, wide enough to image almost all of the retina, without the need for dilation.
The regular fundus camera produces an image of about 45 degrees, covering the important central area of the retina. Within this central area, the optic nerve, macular, arteries and veins can be observed carefully, helping to detect glaucoma, macular degeneration, hypertensive retinopathy, etc.
A wide area is however missed, notably, the peripheral retina where it is more likely to find retinal holes, retinal tears, retinal detachment, cancer, hemorrhages, other forms of degeneration, etc. Figure C shows the difference in retinal coverage between the two technologies. (the inner circle is taken with the regular fundus camera.)

Contact lenses are listed by the Health Science Authority of Singapore as medical Devices. It is therefore only available on prescription by Optometrist and Medical practitioners. Every contact lens importer , must apply for a license to bring in contact lenses into the country. This ensures that contact lenses imported are of high quality and comply with safety standards.
To ensure you continue wearing your contact lenses safely, both these components must go hand in hand. It is very important to safeguard yourselves from fake branded contact lenses by buying from reputable contact lens practices. I say this because we have had an occasion where fake lenses were identified and confiscated by authorities here in Singapore.

Use in Dyslexia, Irlens Syndrome and Migraine
Have you ever looked at a pattern for example on a carpet and the pattern seems to be doing strange things like vibrating or seems to be moving. To some, Lines of text on a page can produce a similar illusion. . Patients describe he text as moving, lines going through the text,. reading is more comfortable in low light,. Even headaches have been reported. This results in slow readers as the illusion is distracting. This is call perceptual distortion.

The Optos Daytona
The Optos is the latest Ophthalmic diagnostictechnology which enables the eye carepractitioner to visualize almost the entireretina without the need for dilating eye drops.


Optical Coherent Tomograph (OCT)
This instrument helps in the diagnosis of many retinal conditions. Examples are Glaucoma, Macular Degeneration and Retinal Detachments.


Auto Refractometer
This is sometimes used, as a starting point. The prescription obtained from this instrument should not be used to order a pair of spectacles, although there are some exceptions to this rule.


Retinal Camera (Fundus Camera)
This instrument takes a picture of the retina. Many retinal conditions can be observed from the picture, eg: Glaucoma, Macular Degeneration, Diabetic Retinopathy, etc. It is invaluable as a documentation tool as well and makes it easy to follow patients with progressive diseases and to determine if their disease is in fact progressing.


Phoropter
This instrument is used to provide our patients with a comfortable testing environment as well as helping to accurately determine your visual status. Trial frames are heavy and uncomfortable.


Slit Lamp Biomicroscope
This microscope is specially adapted to observe the Cornea and other structures of the eye. It is used to check the cornea before contact lenses are prescribed to ensure that they are no contraindications to contact lens use. It is also used to check your cornea after contact lens use to ensure that the eyes are free from any contact lens complications. It is therefore important that you visit your contact lens practitioner at least every six months for after care visits.


Visual Field Analyser
This instrument is used to check your peripheral vision. During a vision test, only your straight ahead vision is tested. If an abnormal results is seen, further investigations must be carried out to determine the course. Many conditions will produce abnormal results, eg: Glaucoma (only affects peripheral vision in its early stages, leaving normal central vision), Brain Tumors, etc.


Topographer
This is an essential tool for the practice of Orthokeratology and to produce custom made Gas Permeable Contact lenses for abnormally shaped corneas. The instrument produces a plot of your cornea describing the shape of your cornea extremely accurately. It is now possible to match the back curve of the contact lens to that of your cornea producing a lens of optimumt, with the Topograph, Special software and an elaborate manufacturing process. This instrument will also pick up abnormally shaped corneas. Example – Keratoconus

The area coloured in red is steep and buldges out while the blue area is at. This condition is called Keratoconus. Vision will be distorted and reduced. Unfortunately, spectacle lenses or soft contact lenses will not clear the distortion. However, Rigid Gas Pearmeable lenses will.
